Toxins requiring reconstitution and ready-to-use toxins

Some botulinum toxin medicines are supplied as a powder for solution for injection and require reconstitution in accordance with their current product information. Others, including certain ready-to-use presentations, do not require reconstitution. This collection therefore includes both botulinum toxin medicines and associated clinical diluents such as sodium chloride. Product-specific preparation requirements should always be checked before prescribing or administration.

What is Botulinum Toxin?
Botulinum toxin is prescription-only medicines containing botulinum toxin type A. They temporarily reduce muscle activity by inhibiting the release of acetylcholine at the neuromuscular junction.

Different brands have different formulations, licensed indications and potency units. Botulinum toxin units are product-specific and should not be directly converted between different brands.

What is the difference between Azzalure, Botox, Bocouture and Letybo?
Azzalure, Botox, Bocouture and Letybo are different branded botulinum toxin type A medicines.

Although they have the same broad mechanism of action, they differ in formulation, potency units, vial presentations, licensed indications and prescribing information.

Importantly, their units are not interchangeable. Practitioners should always follow the dosing information for the specific product being prescribed.
Are Botulinum Toxin units interchangeable?
No.

Botulinum toxin potency units are specific to each manufacturer’s product and are not directly interchangeable.

For example, Azzalure uses Speywood units, while Botox uses Allergan Units. Bocouture has its own product-specific units.

Always refer to the relevant SmPC when calculating or administering a dose.
What is reconstitution?
Reconstitution is the process of adding the specified diluent to a powdered botulinum toxin vial to prepare the solution for injection.

The appropriate diluent and volume depend on the individual product and vial size. Always follow the current manufacturer’s SmPC.
What diluent is used to reconstitute Botulinum Toxin?
The diluent specified for many powdered botulinum toxin products is sterile 0.9% sodium chloride solution for injection.

However, the required volume can vary between products and vial sizes.

Always check the current product information before reconstitution.
Can I use the same dilution for every Botulinum Toxin?
No.

Different botulinum toxin products and vial sizes may have different recommended reconstitution volumes and resulting concentrations.

The reconstitution instructions for Azzalure, Botox, Bocouture and Letybo should therefore not be assumed to be interchangeable.
How much saline is used to reconstitute Azzalure?
For Azzalure 125 Speywood units, the product information provides specific reconstitution options using 0.9% sodium chloride solution.

For example, the manufacturer’s information describes:

* 0.63 mL of sodium chloride 0.9% to produce 10 Speywood units per 0.05 mL
* 1.25 mL of sodium chloride 0.9% to produce 10 Speywood units per 0.1 mL

Always refer to the latest Azzalure SmPC before preparation or administration.
Does changing the amount of saline change the number of units in the vial?
No.

Changing the volume of diluent changes the concentration of the resulting solution, but does not change the total number of botulinum toxin units contained within the vial.

The resulting concentration should always be calculated and checked carefully before administration.
How should Botulinum Toxin be stored?
Storage requirements vary according to the individual product.

Many botulinum toxin products require refrigerated storage at 2–8°C, while some products have different permitted storage conditions.

Always check the specific product’s SmPC and packaging requirements.
What does fridge-line mean?
Fridge-line refers to products that require controlled refrigerated storage and transportation.

Where a product is classified as fridge-line, appropriate temperature control must be maintained throughout storage and delivery in accordance with the manufacturer’s requirements.
Why is my order delivered in temperature-controlled packaging?
Temperature-controlled packaging helps maintain the required storage conditions during transportation.

This is particularly important for fridge-line medicines, including certain botulinum toxin products.
What should I do if my Botulinum Toxin arrives warm?
Do not automatically assume that the product is unusable.
