What you will need

Have these ready before you apply. The Superintendent Pharmacist verifies every application against the relevant UK regulator before approval.

  • Current registration with a UK professional body (GMC, GDC, NMC, GPhC, or HCPC), with your registration number and expiry date.
  • ID (passport or driving licence) and a clear copy of your registration certificate. PDF or JPG, up to 20 MB each.
  • Anti-wrinkle and dermal filler certificates (both required). Skin booster certificate if it applies to your practice.
  • Your clinic or company name, professional address, and a contact email and phone number.

Private Prescriptions

Private prescriptions are dispensed by Longeva Pharmacy and all prescriptions are dispensed in the UK.

All orders placed with Longeva Pharmacy must correspond to a prescription from a UK prescriber. This can be done online using Signature Rx. Please note, orders cannot be dispatched until a legally valid prescription has been received.

All prescriptions must comply with all UK law and regulations and be signed by a registered healthcare professional, who is either a doctor, a dentist, or a qualified non-medically trained prescriber who has performed a face to face consultation with the patient.

The prescribed items must have been prescribed in accordance with all UK prescribing guidance and where a clinical decision has been made for the prescribed items.

Prescribers must only prescribe with a clinical and medical intention. The products are for use by the named patient only and are not for onward sale under any circumstances.

All prescriptions forwarded to Longeva Pharmacy by the prescriber automatically deem the prescriber to have agreed to our terms and conditions. If we receive a prescription written on a different pharmacy's prescription template, all Longeva Pharmacy conditions will apply.

For the Attention of Prescribers

We would like to remind all prescribers that prescriptions for any aesthetic product should be patient specific after conducting an in-person face to face consultation, and never for the provision of stock.

If prescription writing guidelines are not being followed, we reserve the right to inform the appropriate registration body. The MHRA and other registration bodies governing the practice of healthcare professionals have provided explicit guidance on this matter.
