Profhilo Structura practitioner guide showing superficial facial fat compartments and cannula

Evidence reviewed: September 2026. Profhilo Structura is not simply a stronger version of Profhilo H+L and should not be treated as a conventional volumising dermal filler. It is a 2 mL injectable containing 90 mg of hybrid cooperative complexes of high- and low-molecular-weight hyaluronic acid, developed for age-related changes involving superficial facial adipose tissue and facial contour. Two human clinical studies currently provide useful product-specific evidence: a 22-person case series with follow-up to six months and a 50-person observational pilot study with two treatments one month apart.

Quick answer: Profhilo Structura is intended for facial treatment where ageing involves altered superficial fat compartments and tissue support. Its clinical positioning differs from Profhilo H+L, which is primarily associated with skin bioremodelling, and from conventional HA fillers, which are selected when defined projection or volume replacement is the principal objective.

What is Profhilo Structura?

Profhilo Structura is part of IBSA's Profhilo range and uses thermally stabilised hybrid cooperative complexes of high- and low-molecular-weight HA. The formulation contains 45 mg high-molecular-weight HA and 45 mg low-molecular-weight HA per 2 mL syringe, giving a total HA content of 90 mg.

What is Profhilo Structura designed to treat?

The important distinction is anatomical. Facial ageing is not only a skin problem: superficial and deep fat compartments, retaining structures and skeletal support change over time. Structura has been developed specifically around age-related changes involving superficial facial adipose tissue.

That does not mean every hollow, fold or loss of contour is automatically a Structura indication. Practitioners still need to determine whether the dominant problem is skin quality, adipose change, true volume deficiency, skeletal change or a combination.

Does Profhilo Structura really “restore” facial fat?

This claim needs precision. Manufacturer material and emerging clinical literature describe adipose-tissue restoration or remodelling effects, and published studies have investigated changes in superficial facial fat compartments after treatment. However, this should not be translated into the simplistic claim that Structura permanently replaces lost fat.

A more defensible practitioner explanation is that Structura is designed to influence the superficial adipose-tissue environment and improve age-related tissue and contour characteristics. In the 22-person case series, ultrasound assessment found increased soft-tissue thickness and the reported clinical improvement persisted through six-month follow-up. A separate 50-person observational study reported significant improvements in Facial Volume Loss Scale and Wrinkle Severity Rating Scale scores, with benefit maintained three months after completion of the two-treatment course. These are encouraging findings, but both are relatively small non-randomised studies and several authors were affiliated with IBSA. They should not be interpreted as proof of permanent fat replacement or superiority over established fillers.

Profhilo Structura vs Profhilo H+L: what is the difference?

The products share Profhilo's hybrid HA technology but they are not interchangeable.

Profhilo H+L contains 64 mg HA in 2 mL and is principally used where skin hydration, elasticity, laxity and overall skin quality are the treatment objectives. Structura contains 90 mg HA in 2 mL and has a different rheological and clinical positioning, with treatment directed toward facial structural ageing and superficial adipose compartments.

For clinics already offering Profhilo H+L, Structura creates a distinct treatment option rather than simply another pack size: H+L remains relevant to skin-quality-led presentations, while Structura extends the range into patients whose assessment identifies superficial adipose-tissue change. Profhilo Structura vs dermal filler

Structura is not a conventional volumising dermal filler. A traditional HA filler can be selected for defined augmentation, projection, contour creation or correction of a specific volume deficit. Structura's treatment concept is tissue remodelling rather than depositing a shaped bolus of gel to reproduce structural volume.

This distinction matters during consultation. A patient seeking substantial cheek projection, chin augmentation or correction of a clearly defined structural deficit may require a different treatment strategy. Conversely, automatically treating every age-related mid-face change with conventional filler can overlook changes in skin and superficial adipose tissue.

Practitioners comparing injectable materials may also find Longeva's HA versus non-HA dermal filler decision framework useful.

Which facial areas are associated with Profhilo Structura?

Structura is principally positioned for facial ageing involving the lateral and mid-face superficial adipose compartments. Treatment planning should be based on the manufacturer's current instructions for use, anatomy, individual assessment and appropriate training rather than a generic online injection map.

Terms such as “cheeks”, “mid-face”, “facial hollows” and “jawline” are often used commercially, but they should not imply that every point within those regions is an approved or appropriate injection site. Anatomical diagnosis comes first.

What is the Profhilo Structura injection technique?

Profhilo Structura has manufacturer-defined treatment approaches involving the superficial facial fat layer, and cannula-based administration is associated with the product. Technique, entry points, treatment plane and delivered volumes are clinically important and should be learned from the current manufacturer IFU and recognised hands-on training.

A blog should not replace anatomical training with a universal injection recipe. Facial vascular anatomy varies, and a branded protocol does not remove the vascular and other risks associated with injectable HA products.

How many Profhilo Structura treatments are required?

Product-specific human studies used two treatment sessions approximately 30 days apart. In the 50-subject observational pilot, participants received two injections one month apart and were evaluated three months after treatment completion. The 22-person case series likewise used a second treatment at day 30. Subsequent treatment should be based on reassessment and the current manufacturer protocol rather than an automatic maintenance calendar.

How long does Profhilo Structura last?

There is no single duration that can be guaranteed for every patient. In the 22-person case series, clinical improvement was reported across a six-month follow-up from the start of treatment. The 50-person observational study found treatment benefit maintained three months after completion of the two-treatment course. Those study follow-up periods should not be converted into a guarantee that every patient's result lasts exactly three or six months, and they do not establish nine- or twelve-month durability.

Who may be a suitable candidate?

Potential candidates are adults in whom facial assessment identifies age-related tissue change compatible with the product's intended treatment concept, particularly where superficial adipose alteration contributes to a flattened, depleted or structurally aged appearance.

Patient selection should distinguish this from cases dominated by severe volume loss, skeletal deficiency, significant skin laxity or a treatment objective that is better addressed by another modality.

Who may not be suitable for Profhilo Structura?

Suitability requires individual medical assessment and review of the current manufacturer instructions. Relevant considerations include hypersensitivity to components, active infection or inflammatory disease at the treatment site, pregnancy or breastfeeding where excluded by product guidance, significant medical history affecting treatment risk, previous injectable material in the intended plane and unrealistic expectations.

Practitioners should use the current IFU rather than treating a general online contraindication list as exhaustive.

Is Profhilo Structura a skin booster?

The terminology is imperfect. Structura is HA-based and belongs to the Profhilo family, but describing it simply as another “skin booster” understates its intended tissue target. It is more useful clinically to distinguish the patient's treatment objective: skin-quality bioremodelling, adipose/tissue remodelling, or defined structural augmentation.

Can Profhilo Structura be dissolved?

Structura is HA-based, so hyaluronidase is relevant to HA degradation. However, management of an unwanted aesthetic result is not the same as management of suspected vascular occlusion. Practitioners should follow current product information and complication guidance. Longeva's hyaluronidase and filler-dissolving guide explains this distinction, while our vascular occlusion guide covers emergency management principles.

What are the risks of Profhilo Structura?

As with other injectable HA treatments, expected local reactions can include pain, tenderness, swelling, erythema and bruising. More significant complications are possible. Practitioners must be competent in facial anatomy, aseptic technique, patient selection, recognition of vascular compromise and escalation of complications.

The fact that Structura is described as a bioremodelling treatment rather than a conventional filler does not eliminate injection-related vascular risk.

Profhilo Structura or filler: a practitioner decision framework

Dominant treatment objective Category to consider
Skin hydration, texture and laxity Skin-quality/bioremodelling approach such as Profhilo H+L, where appropriate
Age-related superficial adipose/tissue change Profhilo Structura may be considered after anatomical assessment
Defined projection or volume replacement Appropriately selected dermal filler may better match the objective
Complex multi-layer facial ageing Combination or staged treatment may be required following full assessment

This is a decision framework, not a prescribing or injection protocol. Product choice follows diagnosis—not the other way around.

Why stock Profhilo Structura in an aesthetic clinic?

For an established injectable clinic, Structura can broaden the Profhilo consultation rather than duplicate H+L. It gives practitioners a product to consider when assessment points toward age-related superficial adipose-tissue change rather than predominantly skin hydration or a requirement for conventional filler projection.

This supports a clearer product pathway: H+L for appropriate skin-quality presentations, Structura for appropriately selected adipose/tissue-remodelling presentations, and dermal filler where defined structural augmentation is required. A well-selected range helps the clinic match the product category to the underlying tissue problem.

Frequently asked questions

How much HA is in Profhilo Structura?

Each 2 mL syringe contains 90 mg HA: 45 mg high-molecular-weight and 45 mg low-molecular-weight hyaluronic acid.

Is Profhilo Structura stronger than normal Profhilo?

Structura contains more HA per 2 mL than Profhilo H+L, but “stronger” is misleading because the products have different formulations, rheological characteristics and treatment objectives.

Does Profhilo Structura add volume?

It should not be presented as a conventional volumising filler. Its clinical concept concerns tissue and superficial adipose remodelling. Changes in facial contour may occur without the treatment being equivalent to traditional filler augmentation.

Is Profhilo Structura for the jawline?

Commercial descriptions often reference facial contour and jawline improvement, but treatment suitability depends on the cause of the contour change and the manufacturer's current protocol. It is not a substitute for assessing skin, fat, muscle and skeletal contributions.

What age is Profhilo Structura for?

Chronological age alone is a poor indication. Treatment should be based on the anatomical pattern of ageing and clinical assessment rather than a fixed age threshold.

Can Profhilo Structura and Profhilo H+L be used in the same patient?

They address different treatment objectives, so a practitioner may identify indications for both within an overall treatment plan. Sequencing and combination decisions should follow individual assessment and current manufacturer guidance.

Practitioner takeaway

Profhilo Structura expands the Profhilo concept beyond a simple discussion of hydration. Its 90 mg/2 mL hybrid HA formulation is designed around age-related changes in superficial facial adipose tissue and facial structure. The key to appropriate use is therefore diagnosis of the ageing compartment.

Practitioners should avoid three shortcuts: treating Structura as merely “stronger Profhilo”, describing it as a conventional filler, or promising literal permanent restoration of lost facial fat. The more clinically useful question is whether the patient's dominant problem lies in skin quality, superficial adipose tissue, structural volume—or several layers at once.

Evidence quality and limitations

The current Structura evidence base is useful but still developing. The 2023 study was a retrospective case series of 22 volunteers aged 36–60 years; the 2024 study was a single-centre observational pilot involving 50 healthy female participants. Neither was a large randomised head-to-head trial against conventional dermal filler or Profhilo H+L. Authors affiliated with IBSA participated in both publications. These limitations matter when translating study findings into patient-facing claims.

A 2026 animal study adds mechanistic evidence suggesting changes in adipocyte size distribution, adipose-tissue area and tissue compactness after treatment, but animal findings should not be presented as equivalent to demonstrated clinical outcomes in humans.
